Requests and appointments
Submitting a form is a request to be contacted. It does not guarantee an appointment, a specific provider, treatment eligibility or pricing. Treatments such as Avéli® require a consultation, and individual results vary.
Text messaging (SMS) terms
Our SMS program sends messages about your request. If you check the SMS consent box on one of our forms, you agree to receive text messages from Cottonwood Family Medicine, LLC about your request, such as appointment scheduling, reminders and replies to your questions. Consent is optional and is not a condition of purchase or of receiving care.

Insurance and payment
Insurance participation can change. Coverage, co-pays and benefits depend on your plan, so please confirm with your insurer. Co-pays are due at the time of service.
